TIPS

When making an appointment schedule it as far in advance as possible and specify the nature of your visit / problem so we can schedule an appropriate amount of time for your visit.

Bring the following items with you on your visit:
  • Any medications you are currently taking
  • Your insurance card and / or Medicaid or Medicare ID card

    Please advise us of any changes in your address, telephone number, insurance coverage, marital status, etc., that might have occurred since your last visit.

    If you are a new patient you can print and fill out the the patient information forms and bring them with you to your office visit. This will cut down time waiting and allow you to more accurately answer any questions etc.

    If your physician recommends a follow-up visit or tests, please schedule these with the receptionist before you leave our office. This will allow us to answer any questions about additional care you might have.
